The Dane SOS project is part of a community wide effort to end “Unsheltered Homelessness” and is funded for 3 years from HUD (2024-2026) through a competitive process the Dane County Homeless Services Consortium applied for. The position is hosted at Madison Street Medicine which is housed in the Social Justice Center, but it is a project of the Dane County Homeless Services Consortium and the Outreach Committee. We developed a community plan to end unsheltered homelessness and the coordinator is a piece of that strategy. The agencies that get referrals form the Dane SOS hotline and other contacts are:
